Proof of Florida Residency
*** For Hybrid Students only ***
Please note, per our Admissions requirements that are printed on our website and in our Academic Catalog for all out-of-state students, we will need Proof of Florida residency (lease, utility bill, government-issued ID).
Florida Residency Requirements: All students of DRCOM must maintain a continued Florida residency for the duration of the program. The student applicant must provide proof of residency from the acceptable Florida state forms of ID (Driver's License, Utility bill, lease agreement, etc).
With that stated, please provide one of the following for proof of residency in Florida:
- Rent or Lease agreement
- Florida Voters Registration
- Any utility bill, cell or mobile statement (dated within 60 days)
- A redacted bank statement / Financial Statement
- Medical or Health Bill (dated within 60 days)
- Gov’t Issued ID (military, passport, work ID, etc)
- Automobile payment booklet or Auto Insurance
- Letter from Shelter / Transitional Housing (dated within 60 days)
- Any form of proof that states you have residence in the state of Florida.
*Students who do not have any of the above documents may have the person they reside with complete a Certification of Address form, along with 2 proofs of address. The person the student lives with must be present at the issuance office or must sign a Certification of Address form in the presence of a Notary.
